If you're not already a member of MAAC, you'll also want to fill out their form.

Then, simply attend a meeting at the Burlington Seniors' Centre, and submit your form(s) with payment.

Skycraft Hobbies, located on Plains Road East, is also authorized to accept applications and issue membership cards.
